First Visit

Your initial appointment will consist of discussion of your chief concerns, gathering and reviewing of all pertinent dental and medical histories, a complete oral examination and evaluation of radiographs.

Complex dental histories will require evaluation of the case with mounted diagnostic models, photographs, and possible cone beam cat scans. A second, final consultation, appointment will be needed to discuss all pertinent findings in order to develop a comprehensive treatment plan. Treatment is performed once there is a clear and mutually accepted plan of treatment, and acceptance and agreement with all financial obligations.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable
  • A list of medications you are presently taking

If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays

If your previous dentist has taken x-rays, you may request that they forward them to our office digitally.

We require a recent (within 2 years) full set of digital x-rays for proper diagnosis and treatment planning. We are fully aware and sensitive to your concerns with exposure to radiation and will do our very best to limit your exposure while practicing within the standards of care.
